1. Who we are and what this policy covers
Ulsana is a brand of Malpic, LLC, 1267 Willis Street, Suite 200, Redding, CA 96001. In this policy, "Ulsana", "we", "us" and "our" mean that company. The policy covers the website at ulsana.com. It does not cover the websites we link to, which have their own policies.
2. The short version
- The form is for writing to us. What you type goes to our mailbox by email, and we use it to answer you. Please do not include any patient information.
- We count visits with Google Analytics. It sets two cookies; Google receives the visit data (section 3.2).
- We do not sell or share what you send us. We show no advertising, use no other cookies and run no other analytics.
- You can opt out of the counting by blocking cookies or installing Google's add-on (section 6).
- Questions or requests: support@ulsana.com.
3. Information we collect
3.1 What you send through the contact form
The form asks for your name, your email address and your message, and, if you like, your organization and role. We use them for one thing: to answer you. The message goes to support@ulsana.com by email, and we send you a short confirmation that repeats nothing you typed. Please do not include any patient information.
3.2 What is collected automatically
- Visit counting. We use Google Analytics to count visits and see which pages people open. Google receives the page you are on, the page you came from, your browser and device type and your internet address, which tells it roughly where in the world you are. Google explains how it handles this at google.com/policies/privacy/partners. The analytics script and the numbers it sends go through our own address (ulsana.com/ul-g/), though Google's script can still contact Google's own domains. Nothing you type into the form goes to analytics; one event records that a form was sent.
- The form's counters. The program that sends the form keeps counters for 48 hours, keyed by a scrambled form of your email and internet addresses, so one visitor cannot flood our inbox. It writes down whether a message was sent or refused and how long it was, not your name, your address or your words.
4. How we use information
We use what you send through the form to answer you and to keep the conversation. The counters keep one visitor from flooding our inbox. The visit counts tell us which pages people open. That is all we use it for; nothing you send us is used for advertising.
5. Who we share information with
We do not sell or share what you send us. Two companies handle information for us:
- Amazon Web Services runs the site, the form and its email: it stores the pages and the form's counters and sends the email for us.
- Google receives the visit data described in section 3.2.
We show no advertising, use no other cookies and run no other analytics. The site's fonts are served from our own address, so no font service sees your visit.
6. Cookies
Google Analytics sets two cookies, _ga and _ga_BFCWLTCSMS, which give your browser a random number so one visit is not counted twice. They are the only cookies this site sets.
To opt out, block or delete cookies in your browser's settings, or install Google's opt-out browser add-on. The site works the same either way.
Browser signals. Browsers can send a signal asking sites not to follow you; this site has no way to act on it, so a visit is counted the same with or without it unless you block cookies or use the add-on.
7. How long we keep information
- Your message stays in our mailbox as long as we need it to answer you and keep the conversation.
- The form's counters expire within 48 hours.
- Visit data in Google Analytics: the detailed data for 14 months; the totals in its reports stay longer.
8. Your rights and choices
Email support@ulsana.com to ask what we hold about you, to correct it or to have it deleted. We answer every message ourselves. Section 6 says how to opt out of the visit counting.
9. Children
This site is for people who work at treatment centers; we do not knowingly collect information from anyone under 18.
10. Changes to this policy
Each version of this policy has a number and an effective date, shown at the top. When the policy changes, the new version appears here.
